Keane is the only guard with Andy in the armoured van while going to Merrick's, and he seizes the opportunity to fuck her exactly how he likes it - bonded/gagged or not, consenting or not, with Booker watching or not, trying to impregnate her or not... all this is up to the writer. At the end of the scene, the reader discovers it was just Keane's fantasy/daydreaming.
